| Registration and Immunization | |
Registration: To register your child in school, the following documentation is needed: 1. Birth Certificate 2. Proof of immunization on Form 680, which can be obtained at the Department of Health: Orange County Health Department
3. Proof of physical examination within the last year. If documentation cannot be provided, a physical examination must be scheduled within 30 days. 4. Verification of your residency in Orange County with one of the following: A. Homestead Exemption Card
To obtain a Declaration of Domicile ($8.00), apply at the Comptroller Office, Monday through Friday, 7:30a.m - 4:30 p.m.: 400 S. Rosalind Avenue
Foreign Students: Foreign students who wish to change their status from non-student should contact Virginia Redding, Foreign Student Advisor, Pupil Assignment, at 407-317-3200 Extension 2114.
Immunization Requirements: 6th Graders: Your child must be given their Hepatitis B Series (2 or 3 shots depending on series) AS SOON AS POSSIBLE, as it will take a minimum of 4 months to complete their 7th grade immunization requirements. Immunization requirements are: Hepatitis B

7th and 8thGraders: If your child entered school and had just begun the Hepatitis B Series, it is your responsibility to finish the series on time. Your child will not be allowed to enter the next grade level without these required immunizations.
